Medication can be issued for most of our patients by the dispensers at our two main surgeries. However if you live within one mile of an independent Pharmacy we are not permitted to dispense to you.

If you are taking regular medication you can obtain repeat prescriptions through the dispenser.

Prescriptions for oral contraceptives will appear on the repeat form but cannot be dispensed until you have had a 6 monthly pill check appointment with a Practice Nurse.

Either Order Your Prescription Online:

	Method 2 - via our online form here » You must fill in the patient number which is quoted on the top right hand corner of the repeat slip (the tear off section) and your date of birth.

Or

Use the repeat slip supplied by the dispensary, ticking the items required, and put this in the box in the waiting room.

Or
 

Fax your medication request to the dispensary:           FAX HOLBROOK: 01473 327185   FAX SHOTLEY: 01473 788386

Or

Telephone the dispensary after 12 noon. Please use this option only if no other is convenient to you.

Please Note: allow 48 hours for the prescription to be made up.

You will be required to see a Practice Nurse or doctor at intervals to ensure that the medication you are taking is still appropriate for your needs.
