Please note:  we are introducing some changes to how you can order repeat prescriptions online.  Click here to read more.

Please note that we do not accept Repeat Prescription requests by email.

This online form is the only accepted method for receiving online Repeat Prescriptions.

All the information you need to complete this online form can be found on the tear off paper Repeat Prescription form issued to you by your Doctor. It is important that we receive a fully completed online form containing the correct data in the order listed on the form otherwise a Repeat Prescription cannot be issued.

We can only accept Repeat Prescription requests for items listed on your paper Repeat Prescription form; other items cannot be dispensed.

If you wish to query or change any details, please telephone the Dispensary after 12 noon, Holbrook 01473 327571, Shotley 01473 788673.


Online requests received before 12 noon Monday-Friday will be available for collection within 2 working days. Orders received after 12 noon may not be available for collection for a further 24 working hours. You should also take into account that Holbrook Dispensary is closed on Thursday afternoons and Shotley on Friday afternoons.

You can collect your medicines at anytime the surgery is open. See Opening Hours on the Home Page.

Please note that you may order repeat prescriptions up to 2 weeks in advance of your present prescription running out.

Prescriptions for oral contraceptives will appear on the repeat form but cannot be dispensed until you have had a 6 monthly pill check appointment with a Practice Nurse.

Your Patient Number
Your patient number is the five digit number on the top right hand corner of the tear off section of your prescription, which lists your medication. In order to protect your confidentiality, as the email system is not always secure, we do not require your name or address.  If your patient number only has four digits, please add a zero in front of it.
