REGISTERING WITH THE PRACTICE
Please register in person if possible, bringing with you your NHS
number.
You will be asked to complete 3 forms and will be encouraged to
make an appointment with a nurse for a new patient medical check.

All 3 forms must be completed and the GMS1 must be signed. They can be
posted or brought to the Surgery in person.

TEMPORARY RESIDENTS
Visitors to the area who fall ill can be treated by the Practice but
will need to register as temporary residents.
YOUR OWN DOCTOR
Although you will be registered with the Practice rather than an
individual GP, it will normally be possible for you to see the doctor of
your choice.
If he/she is absent for any reason, or their clinic is fully booked,
the receptionist will arrange for you to see another doctor.
As the Practice offers evening appointments at only one surgery, please
be prepared to attend either building and consult the doctors who are
holding clinics there.

If you are moving house within our practice area, or you change your

When completed, the form should be signed and brought or posted to
the surgery so that we can ensure we have current details for you.
