New Patient Registration
NHS England requires practices to utilise the GP2GP facility for the transfer of patient records between practices, when a patient registers or de-registers (not for temporary registration). It is very important that you are registered with a doctor at all times. If you leave your GP and register with a new GP, your medical records will be removed from your previous doctor and forwarded on to your new GP via NHS England. It can take your paper records two or more weeks to reach your new surgery. With GP to GP record transfers your electronic record is transferred to your new practice much sooner. We confirm that GP to GP transfers are already active and we send and receive patient records via this system.
Registering With the Practice
Please register in person if possible, bringing with you your NHS number.
You will be asked to complete the forms below and will be encouraged to make an appointment with a nurse for a new patient medical check.

Visitors to the area who fall ill can be treated by the Practice but will need to register as temporary residents.
Your Own Doctor
Although you will be registered with the Practice rather than an individual GP, it will normally be possible for you to see the doctor of your choice.
If he/she is absent for any reason, or their clinic is fully booked, the receptionist will arrange for you to see another doctor.
As the Practice offers evening appointments at only one surgery, please be prepared to attend either building and consult the doctors who are holding clinics there.
Change of Name or Address
If you are moving house within our practice area, or you change your name, please download and print our Change of Details Form HERE.
When completed, the form should be signed and brought or posted to the surgery so that we can ensure we have current details for you.
