CONFIDENTIALITY AND THE DATA PROTECTION ACT
All information held by the Surgery and Staff is strictly confidential
and is held for the purpose of medical care. We are unable to divulge
any information about a patient to another person, except in the case of
young children to their parents.
Patients may ask to access their medical records. The request must be
made in writing and a charge may be made.
The Doctor has the right to withhold information which is considered to
be potentially harmful to the patient, or if information concerns a
third party.
USE OF PATIENT DATA FOR MANAGEMENT PURPOSES
It may be necessary to transmit some patient data to the Primary Care
Trust for management, payment or audit purposes. This is likely to
concern special treatments or procedures such as minor surgery, minor
injury treatment or special tests and injections. Please tell the doctor
or nurse BEFORE the procedure if you do not agree to this transfer of
information.
SUFFOLK PRIMARY CARE TRUST
All NHS primary care services such as doctors, community nurses,
dentists, opticians etc and all services obtained through hospitals are
co-ordinated by Suffolk Primary Care Trust. You can contact them at

VAT
As a Dispensing Practice, following Government guidance, we have
registered for Value Added Tax, Registration Number 879 0816 77. As a
result, VAT @ 17.5% will be added to the cost of certain private fees
and to private prescriptions.
YOU CAN ASSIST US BY
- Letting us know if you are unable to attend an appointment, even
if it is only a short while before.
- Attending surgery if at all possible rather than calling a
doctor out.
- Talking to our reception staff and accepting the advice and
assistance given. They are extremely experienced and work according
to guidelines given to them by the doctors. They can often answer
queries and give general help.
- At the direction of the reception staff, speaking to one of our
Practice Nurses for advice where this is appropriate.
- Informing us in writing of any change of name, address or phone
number.
- Remaining courteous to our staff – in fact we will not tolerate
violent or abusive behaviour. In such incidents the Police may be
called and the patient’s medical care may be referred to the Violent
Patients Scheme which operates from premises in St Clements
Hospital, Ipswich.
SURPLUS MEDICAL EQUIPMENT and WHEELCHAIRS
The practice is able to make short term loans of wheelchairs and
other items of equipment to patients, usually for the price of a small
deposit. Patients should be aware that during such loans they are
responsible for the item.
If you are having a house clear out and come across a wheelchair or
other medical equipment which is labelled ‘Property of Holbrook/Shotley
Surgery’, please return it without delay to your local surgery.
If you have an unused NHS wheelchair (but it is not the property of the
local surgery), please contact the Wheelchair Service on 01473 692727
If, however, you come across NHS crutches, walking aids, commodes,
frames or hoists, please contact the Central Equipment Stores (01473
274804) and arrange for collection. Unfortunately we can not
accept them at the doctors surgery as we have no space for even short
term storage.
With regret, we cannot accept donations of privately owned surplus
equipment and suggest you contact The Red Cross on 01284 767 215. They
are happy to take, and find good uses for, wheelchairs and other items. |
